Liability Release

I, the undersigned participant (or parent/guardian, if participant is under 18 years old), acknowledge and fully understand that participation in the Liberty Dance Championship involves physical activity that carries inherent risks, including but not limited to injury, illness, or other damages.

By signing this waiver:

• I agree to assume all risks associated with participation in the event, including those arising from negligence.

• I release and hold harmless Liberty Dance Championship, the event organizers, Harrah’s Casino, their employees, contractors, agents, and volunteers from any and all liability, claims, demands, or causes of action that may result from my participation in the event.

Media Release

I understand that Liberty Dance Championship will be recorded, photographed, or otherwise documented for promotional, educational, or commercial purposes. By signing this form, I:

• Grant Liberty Dance Championship and its affiliates permission to use my name, image, voice, and likeness in any promotional materials, including but not limited to social media, websites, advertisements, and print publications.

• Waive any rights to review, approve, or receive compensation for the use of such materials

Health and Safety Acknowledgment

I certify that I am in good health and physically capable of participating in the Liberty Dance Championship. I agree to notify event staff of any injuries, allergies, or medical conditions that may affect my participation.

I also agree to abide by all rules, safety protocols, and guidelines set by the organizers and venue staff to ensure a safe event environment.
